Case Management

Case Management is a no-fee service the Senior Center provides to Verona seniors, as well as, their families and caregivers. Case managers can assist with one time needs or be available for ongoing support and can assist or make referrals for the following needs:

 

  • Nutrition meals (home-delivered and Grab & Go)
  • Transportation (in Verona and to medical appointments)
  • Medicare, Medicaid, Medicare Part D, and other government programs
  • Housing
  • Personal / in home care
  • Caregiver resources and support
  • Elder abuse and neglect
  • Hospice and end of life
  • Taxes (referrals only)
